If you've ever made a cover letter for a package of information, it's the same thing.
This is the sheet that goes in front of your package of declarations from yourself and your parents/guardians - a title page if you will. You should be listing exactly what is in this package, so something like this:
1. Supporting Documentation - Student Signatures:
x pages
2. Supporting Documentation - Parent Signatures:
x pages
They're just looking for a summary of what is in your package of information. Doing something like the above works fine
